Prizes in the raffle have been donated by friends of Chemo Comfort. No substitutions or exchanges can be made. Donor terms and conditions apply. Descriptions of prizes and retail values have been provided by the donors and are subject to change.  Chemo Comfort assumes no responsibility for the accuracy of any description. In the case of prize trips, all of which are non-transferable and subject to availability, certain donor terms and conditions may apply. Chemo Comfort assumes no responsibility or liability whatsoever for any property loss, damage, personal injury or death which might occur during a trip, outing or service.  Winners will be determined in random drawings to be held on May 19, 2009. All prizes will be awarded. The drawings will be supervised by officers of Chemo Comfort. Their decision will be final. Your ticket stubs (large portion which you retain) and cancelled check are your receipt for tax purposes. Winners will be notified by phone or e-mail. If we are unable to contact you with the information you provide us or if you do not accept your prize, the prize will go to an alternate winner. Entries must be postmarked on or before May 11, 2009 to be included. A list of prize winners will be available on our web site after May 19, 2009. Officers and directors of Chemo Comfort, as well as their spouses and children, are not eligible. Any taxes on any prizes are the sole responsibility of the prize winner. Odds of winning depend on the number of entries. Prize winners release Chemo Comfort and its agents from any and all liability with respect to the receipt or use of any merchandise, service or travel prize. Chemo Comfort is not responsible or liable for any warranty, representation or guarantee regarding the quality, mechanical condition or fitness of the prize, nor is it responsible for malfunction, fit, availability or delays in delivery of prizes.
